However, skeletal system development is just as important and should be regularly monitored. Leg length discrepancies that go unnoticed, especially during childhood, can lead to serious orthopedic problems in later life.

According to orthopedics and traumatology specialists, if leg length discrepancy is not detected early, problems such as spinal curvatures, early osteoarthritis in hip and knee joints, and chronic pain can develop. Therefore, early diagnosis is crucial.

What is Leg Length Discrepancy?

Leg length discrepancy is a condition where there is a measurable difference in length between the two legs. In mild cases, it may often not show symptoms. However, as the difference increases, noticeable changes may appear in daily life.

One common misconception is the belief that this condition will correct itself as the child grows. However, especially in cases related to growth plate (epiphyseal) damage, the difference can become even more pronounced as the child grows.

What Symptoms Should You Look Out For?

Children with leg length discrepancy may exhibit the following symptoms:

Limping or an uneven gait when walking
Favoring one leg
Uneven wear on shoe soles
Asymmetry in shoulder and hip levels
Postural abnormalities

If these symptoms are present, an orthopedic evaluation is essential.

What Causes Leg Length Discrepancy?

This condition can be congenital or develop later in life. The most common causes include:

Fractures affecting the growth plate
Infections
Developmental hip dysplasia
Congenital limb development differences
Genetic and syndromic diseases
Rarely, bone tumors and rheumatic diseases

These factors can prevent bones from growing equally.

Non-Surgical Treatment for Mild Cases

Not every leg length discrepancy requires surgical intervention. Conservative methods are generally preferred, especially for differences under 2 centimeters.

Custom Orthotics

Orthotics used to support the shorter leg help maintain body balance. This reduces the load on the spine and joints.

Shoe Lifts

In more pronounced but non-surgical cases, balance can be achieved with lifts added to the shoe sole.

Surgical Methods for Advanced Cases

Surgical options become relevant when the discrepancy exceeds 2–2.5 centimeters. Today, the goal of these surgeries is to achieve body symmetry by balancing bone length.

External Fixators

Controlled lengthening is performed using metal frames fixed externally to the bones. This method is particularly preferred in cases accompanied by deformity.

Magnetic Lengthening Systems

Controlled lengthening can be achieved without external intervention using magnetic nails placed inside the bone. This stands out as a more comfortable and aesthetic method.

Combined Methods

In some cases, internal and external systems can be used together to reduce treatment duration and the risk of complications.

Epiphysiodesis (Growth Control)

The goal is to slow down the growth of the longer leg in a controlled manner, allowing the shorter leg to catch up over time.
