Description of the process

Breast augmentation aesthetics (augmentation mammoplasty) is a surgical operation performed by inserting silicone implants into the breasts to optionally change the volume and shape of the breast. Thanks to this process, small or lost volume nipples can be made more plump and shaped. The silicone gel implants used are body-compatible, durable materials and can be used safely for many years. The main goal of breast augmentation surgery is to obtain breasts of natural appearance and desired size, positively affecting the patient's body proportions and self-esteem.

How Is It Done?

Breast augmentation surgery is a procedure that usually takes about 1-2 hours, performed under general anesthesia. The surgeon prepares the pocket area where he will place the implant by making incisions in the breast according to the agreed plan. Incisions can usually be made in the under-nipple fold, in the dark ring around the nipple (areola) or in the armpit area, so that the scars that will occur are hidden as much as possible. The prepared pocket is the space inside the breast tissue where the implant will be placed. Implants can be placed under the breast tissue or under the pectoral muscle (pectoral muscle). Which plan to use is determined by the patient's available amount of breast tissue, body structure, and surgeon's preference. For example, in people who are very weak and have little breast tissue, the implant is usually placed under the muscle, providing a more natural inclination and softness.

The surgeon carefully inserts silicone implants of the size and shape selected during the operation into both breasts and makes sure that a symmetrical appearance is obtained. Then the incisions are closed with thin stitches. At the end of the operation, a special bandage or elastic bandage is applied to the patient to support the new shape of the breasts and reduce swelling. In some cases, small drain tubes may be inserted into the nipples to prevent excess blood or fluid buildup after surgery; these drains are usually withdrawn within 1-2 days. After breast augmentation surgery, the patient is usually discharged on the same day or after 1 night in the hospital.

Who is it suitable for?

  • Adults Completed Breast Development: For surgery, the patient's body development must be complete. Usually people who have passed the age of 18 and have completed the maturation of breast tissue can be candidates.

  • Nipples Experiencing Small or Volume Loss: Those who have a naturally small breast structure, or who experience volume loss and sagging in their breasts after pregnancy, breastfeeding, or severe weight loss.

  • Those Who Have Asymmetry In Their Breasts: Patients who have a marked difference in size or shape between the two breasts and want to correct it. With the implant, one breast can be brought to the same size as the other.

  • What's in Place of Physical Health: People who do not have a serious chronic illness, uncontrolled health problems, conditions that do not have obstacles to surgical anesthesia. In addition, a breast examination before surgery should not detect any mass or active disease.

  • Those with realistic expectations: People who have realistic expectations about the outcome of surgery, who have made informed decisions about their new appearance. Breast augmentation can increase self-confidence, but it should not be done with the expectation of perfection.

  • Non-Pregnant and Non-Breastfeeding: This operation is not recommended during pregnancy or in the process of active breastfeeding. After the end of these periods and the cessation of milk production by the breasts, surgery should be planned.

As with any aesthetic operation, in breast augmentation surgery it is important that the candidate is psychologically motivated and determined. The surgeon will decide whether surgery is appropriate by assessing the patient's expectations, body structure, and health history at the initial examination. Possible Risks and Complications

  • Infection: Infection can develop at the incision site or around the implant, although rarely after surgery. In such a situation, antibiotic therapy is prescribed, in severe infections, temporary removal of the implant may be required.

  • Hemorrhage and Hematoma: Bleeding may occur at the site of the operation and blood accumulation (hematoma) may appear. Therefore, after surgery, drain use and close monitoring is performed. In the presence of pronounced hematomas, it may be necessary to clean them with additional surgical intervention.

  • Capsule Contracture: The body forms a connective tissue capsule surrounding the inserted implant. In some patients, this capsule thickens and hardens over time, which can compress the implant and lead to stiffness, deformity, or pain in the breast. If capsule contracture occurs, it may be necessary to surgically loosen the capsule or replace the implant.

  • Implant Leakage or Rupture: Modern silicone implants are quite durable, but they can rarely be damaged, leaky or torn. If saline implants leak, the breast will soon return to its former size; in silicone gel, the leak usually remains inside the implant capsule. The problem with implant damage is usually resolved by replacement surgery.

  • Sensory Changes: In the early postoperative period, there may be sensory changes such as numbness or excessive sensitivity in the nipple or breast skin. This condition is mostly temporary and returns to normal within a few months. In rare cases, permanent change of sensation can occur.

  • Scar and Aesthetic Results: Depending on the condition of the incisions, a thin scar may remain under the breast or around the areola. The severity of the scars depends on the skin structure of the person and the healing process. Some people are at risk of developing hypertrophic scars or keloids (abnormal scarring). It should also be borne in mind that revision surgery may be required if asymmetry or an aesthetically unsatisfactory result occurs outside the expected.

  • Anesthesia and Other Risks: Adverse reactions due to general anesthesia (such as allergies, respiratory problems) can occur, although very rarely. In addition, the risk of clot formation (deep vein thrombosis) in the vein due to long immobility is present, as with any surgery. That is why it is important to act in the early period after the operation and, if necessary, use blood-thinning drugs.

Most of these complications are unlikely to occur, and serious problems are not expected by an experienced plastic surgeon in surgeries performed in a fully equipped hospital. Nevertheless, the patient should pay attention to the warning signs and contact his doctor if he feels any problems.

Healing Process

Although the recovery process after breast augmentation surgery can vary from person to person, it generally follows:

  • The First Days: After surgery, swelling, tightness, and pain may be felt in the chest area. This is normal and is controlled with pain medication given by your doctor. The breasts will be bandaged or in a special surgical bra. The first few days it is necessary not to strain the arms excessively and to rest.

  • First Week: In the first 5-7 days after surgery, there may be increased sensitivity, especially with arm movements. During this time, your doctor will most likely remove the bands that support the suture line and any drains, if any. The patient is able to return to his daily work, which does not require heavy physical exertion, usually within a week. However, the time to return to work varies depending on the nature of the work done; those who do desk work can return earlier, while those working in jobs that require physical exertion are advised to wait a little longer.

  • The First Few Weeks: For about 4-6 weeks, it is important not to lift heavy, do not exercise intensively, and avoid movements that require lifting the arms up. During this period, you should help fix the implants in the correct position by using the supportive sports bra recommended by your doctor without interruption. Swelling and light bruises disappear from the second week, decreasing.

  • After 6-8 Weeks: Usually 6-8 weeks after surgery, the body is greatly recovered. After this stage, the normal exercise routine and more strenuous activities can be gradually started with the approval of the doctor. Implants may lie higher and taut in the chest at first; however, within weeks they will fall into place and take on a more natural appearance.

  • Long Term: It can take several months to a year for the scars to mature and silicate. In the process, methods such as protection from direct sunlight and using recommended scar creams can improve trail quality. Modern implants are quite durable, but they are not lifelong devices; they may need to be replaced usually after 10-15 years or if any problems arise. Therefore, it is important for patients to have their routine check-ups in the long term.

Patients should not interrupt their postoperative control and follow the advice of their doctor. A health care provider should be consulted immediately if symptoms such as unusual stiffness, excessive swelling, redness or fever occur, especially in the first months.
